Group Health Coverage
You are eligible to participate in programs which provide medical, dental and vision care. You may elect each benefit option independently, and dependent coverage is available. While your employer pays for most of the cost of this coverage, you are required to contribute a portion of the cost of coverage elected on a bi-weekly pre-tax basis.
Voluntary Short-Term Disability Coverage
You may elect voluntary short-term disability coverage. This coverage can provide benefit payments of up to 60% of your salary to a maximum of $2,000 per week. For certified disabilities, benefits typically begin on the 15th day of illness or injury and may be paid for up to 11 weeks.
Long Term Disability Coverage
Your employer pays the premium for group long-term disability coverage. This policy will provide a benefit of up to 60% of total cash compensation (salary plus bonus), up to $15,000 monthly maximum benefit, if you are disabled (typically that you are unable to perform the essential duties of your own occupation) after a 90-day elimination period. Benefits are reduced by certain income provided by your employer or other sources (i.e., workers’ compensation, social security, etc.).
Life/Accidental Death & Dismemberment Coverage
Your employer pays the premium for group term life insurance coverage at the multiple of your annual salary as specified below:
Business Travel Accident Coverage
In the event of your accidental death while on your employer’s business, this coverage will provide your beneficiary(ies) with a multiple of your annual earnings. Dismemberment and paralysis benefits are also included
Medical and Child Care Flexible Spending Accounts (FSAs)
You may defer up to $2,550 in pre-tax dollars to a flexible spending account (“FSA”) which may be used for unreimbursed Internal Revenue Service (IRS) approved health care expenses incurred during a calendar year. Employees may also elect to defer up to $5,000 in pre-tax dollars into a FSA which may be used for IRS approved dependent child care and, in certain circumstances, adult day care.
Holidays
Immediately upon employment, you are eligible for the following paid holidays in addition to the accrued PTO; however given the nature of medicine, you may be required to cover call on holidays based upon a rotational schedule. Compensatory PTO is not offered for worked holidays.
Sick Leave
One sick leave day per calendar month is earned (12 days or 96 hours per year) until 480 hours have been earned. Unused sick leave may be carried over for use in a later year. If you fall below 480 hours, you may earn sick pay again until you are back at 480 hours. Borrowing against future sick leave for taking paid sick days (i.e., a negative balance) is not permitted. Sick leave is only to be used during employment and may not be cashed out.
